FIRST APPOINTMENT
If you would like to request your FIRST appointment simply take these 3 steps:

1) Please read and agree to the "Contract" and "Before, During and After Your Sessions" pages.

2) Pay for your first appointment using PayPal.

3) Contact me to schedule a mutually agreeable appointment time.

FOLLOW UP APPOINTMENTS
After your first appointment, when you are ready to request a follow up appointment or "The Package", here's how to proceed:

1) Pay for your Follow up Appointment or The Package PayPal online or Money Order by mail.

2) Mail a recent photo (head-to-toe if possible) of yourself to me along with copies of any relevant x-rays or medical images. (Photos and medical images will be kept in your file, not returned, so please send copies only. Please ask me for the mailing address.)

3) Contact me to schedule mutually agreeable appointment times.
